BG3 Volo Eye Surgery: Should You Let Volo Remove Your Eye? Full Guide

How It Works

1. Rescue Volo from the Goblin Camp (he’s being forced to perform for the goblins)
2. He joins your camp
3. Talk to him at camp. He offers to remove the tadpole from your brain with “ancient dwarven techniques.”
4. Let him operate. It goes… poorly. He removes your eye.
5. Volo flees (he’ll return). You get a permanent magical replacement: Volo’s Ersatz Eye.

The Buff: See Invisibility

Volo’s Ersatz Eye gives you permanent See Invisibility. Any invisible creature within 9 meters becomes visible to you. This is extremely useful throughout the entire game — invisible enemies, traps, and secrets are revealed automatically.

Is It Worth It?

Yes. The cosmetic change is minor (slightly different iris color). The permanent buff is strong and saves spell slots. There’s no combat penalty, no stat loss, no companion disapproval. It’s one of the best permanent buffs in the game.

When to Get It

Get it as early as possible (Act 1, after rescuing Volo from the Goblin Camp). The buff is useful throughout the entire game.
